Reconstructive eyelid surgery
Reconstructive eyelid surgery restores normal eyelid position and protects the surface of the eye. Ectropion is a condition in which the lower eyelid droops and turns outward. The eyelid can no longer protect the eye, so it is easily exposed to environmental factors and inflammation. To keep the surface moist, the eye must produce a large amount of tear fluid. Repeated infections can damage the cornea and, in severe cases, lead to loss of vision.
Reconstructive eyelid surgery for ectropion
During ectropion repair, the eyelid is shortened and anchored to the supporting tissue at the outer corner of the eye. In some cases, the skin of the lower lid is also deformed.
A piece of healthy skin taken from the upper eyelid on a two-vessel pedicle can then be transplanted in its place. The operation is performed in two stages. First, the lower eyelid defect is corrected. After two to three weeks, the edge of that flap is separated near the inner corner and smoothed.

Entropion: when the eyelid turns inward
Entropion is the opposite problem: the eyelid turns inward, toward the eyeball. The lashes rub against the eye surface, causing irritation, tearing, and recurrent inflammation.
During the operation, excess skin and scar tissue are removed. The inner lining of the eyelid can be rebuilt with a graft of oral mucosa.
Reconstruction after tumor removal or major tissue loss
If the eyelid has to be removed completely because of a tumor, it can be rebuilt with a local skin flap. The supporting cartilage, called the tarsus, is replaced with a graft taken from the nasal septum or the auricle of the ear.
A smooth mucosal graft from the inside of the cheek is also used so that the new eyelid does not irritate the cornea.
